Before our first meeting, we kindly request that you complete our intake form. This includes your consent for counseling and helps us understand some basic information about you.

• If you need to reschedule, kindly inform us at least 48 hours in advance.
• Sessions are limited to four per day to ensure quality and presence—last-minute cancellations impact others who may be waiting.
• To avoid last-minute cancellations and to create accountability, payment for sessions is to be made before the session begins. Please share your payment confirmation once done.
